CPV Description: Company provision of comprehensive service to the employees of the Social Assistance Agency and the Territorial and Occupational Health Service Employees The subject of the contract covers the provision of a comprehensive service to the employees of the ASA and its territorial units by the Occupational Medicine Service, of Art. 25 of the Health and Safety Conditions nLabor Code, Ordinance No. 5/1999 on the order, manner and periodicity of carrying out risk assessment, Ordinance № 3/2008 on the conditions and the order for performing the activity of the occupational medicine services and other relevant normative acts . The functional scope of the service is detailed in the Technical Specification - Attachment from the Procurement Documentation.• The subject of the procurement includes the provision of a comprehensive service to the employees of the Social Assistance Agency and its territorial units by the Labor Medicine Service, in accordance with the requirements of Art. 25 of the Health and Safety at Work Act, Ordinance No. 5/1999 on the order, manner and periodsthe risk assessment, Ordinance № 3/2008 on the conditions and the order for performing the activity of the occupational medicine services and other relevant normative acts, as follows:

1. Develop measures to improve working conditions and eliminate and reduce the risk to health and safety at work; Employee health monitoring and analysis-in relation to the work performed, the working conditions and the fitness for work; Develop a program and conduct training of employees and officials on the rules for health and safety at work; Organizing and conducting specialized measurements and assessments of the working environment and labor and occupational safety; Production of personal health records-the health status of the employees; Treatment of hospital sheets for temporary and permanent disability and performance of morbidity analysis with temporary incapacity; Annual health status analysis of employees on the basis of the results of preventive medical examinations carried out; Organizing and conducting medical examinations of assigning employeesaccording to the requirements objectified in the PPL documentation; Measurement of the working environment factors; Concluding a conclusion on the suitability of the employee to perform a certain type of work. The conclusion is drawn up on the basis of normative acts to ensure health and safety at work, the risk assessment and the results of the medical examinations and studies carried out in accordance with the modelpursuant to Ordinance № 3 / 25.1.2008.

Participants must hold a valid registration under the procedure of Art. 25c of the Health Protection Act - valid registration certificate under Art. 25d, para. 3 of the Health and Safety at Work Ordinance, issued by the Minister of Health - is declared in the enEEDOP by indicating the number, the issuing authority, the year of issue, the term of validity and the subject (ENEDOP - Part IV, Section A).-for the economic and financial status of the participants.

Other selection criteria related to personal status:

1.1. No participation in the procedure is allowed and a participant for whom any of the circumstances under Art. 54, para. 1, item 1, 2, 3, 4, 5, 6 and 7 and under Art. 55, para. 1, item 1 of the Public Procurement Act. Participant for whom the grounds under Art. 54, para. 1 of the Public Procurement Act and the specified circumstances under Art.55, para. 1, item 1 of the PPA has the right to provide evidence that it has taken measures that guarantee its reliability, despite the relevant justification for removal, according to the procedure envisaged in the PPA

Statement of circumstances under Art. 59, para. 1, item 3 of the Measures against Money Laundering Act and under Art. 3, item 8 of the Economic and Financial Relations with Companies Act, registersin jurisdictions with preferential tax regime, the persons related to them and their actual owners
