Subjectthe purpose of the contract is to provide cleanliness services in common parts of residential buildings, maintain cleanliness and care of the greenery of outdoor areas adjacent to buildings and non-residential plots. AMW. A detailed description of the subject of the contract can be found in Annex 1 to the ToR. The order was divided into 2 parts:

Main Site: Gdynia, Pruszcz Gdański, Gdańsk, Malbork, Hel, Wejherowo.

The subject of the contract is the provision of cleaning services in common parts of residential buildings, maintenance of cleanliness and maintenance of greenery of external areas adjacent to buildings and plots of undeveloped AMW plots.

Part 1:

Provision of maintenance services partproperty in parts of common residential buildings located in Gdynia, Pruszcz Gdański, Gdańsk, Malbork, and Hel. Maintaining cleanliness and care of the greenery of outdoor areas adjacent to buildings and undeveloped plots of land located in Gdynia, Pruszcz Gdański, Gdańsk, Malbork, Hel, Wejherowo.

1. In accordance with art. 13 section 1 and 2 of the Eu Parliament Regulationof the European Union and Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (General Data Protection Regulation) (OJ EU L 119, 4.5.2016, p. 1), hereinafter referred to as "GDPR", I inform you that: the administrator of your personal data is the Military Property Agency withbased at ul. Nowowiejska 26 A, 00-911 Warsaw.

2. The administrator has appointed a Data Protection Inspector with whom you can contact in matters related to data protection by e-mail to the following address: iod@amw.com.pl or by post to the address of Agencja Mienia Wojskowego, ul. Nowowiejska 26A, 00-911 Warsaw, POLAND, with the note "Data Protection Inspector.

3. Your personal datathese will be processed on the basis of art. 6 clause 1 lit. c GDPR for the purpose related to the procedure for the award of a public contract.

4. The recipients of your personal data will be persons or entities to whom the documentation of proceedings based on art. 8 and art. 96 section 3 of the Act of 29 January 2004 Public Procurement Law (Journal of Laws of 2017, item 1579 and 2018), hereinafter "awa Pzp ".

5. Your personal data will be stored, in accordance with Article 97 (1) of the Pzp Act, for a period of 4 years from the date of termination of the contract award procedure, and in the case of the content of the contract for a period of 10 years or longer in the case of a contract concluded for a period of over 10 years, then for its duration.

6. Your obligation to provide your personal data directly regarding you ist the statutory requirement specified in the provisions of the PPL Act related to participation in the public procurement procedure; the consequences of not providing specific data result from the Public Procurement Law

7. With regard to your personal data, decisions will not be taken in an automated manner, applying to art. 22 GDPR.

8. You have:

a. based on Article. 15 GDPR, the right to access datapersonal data concerning you;

b. based on Article. 16 GDPR, the right to rectify your personal data (exercising the right to rectification may not result in a change in the outcome of the public procurement procedure or a change in the provisions of the contract to the extent that is not in accordance with the Public Procurement Law and may not violate the integrity of the protocol and its annexes);

c. based on Article. 18 GDPR, rightrequests from the administrator to limit the processing of personal data, subject to the cases referred to in art. 18 clause 2 GDPR (the right to limit processing does not apply to storage, to ensure the use of legal protection measures or to protect the rights of another natural or legal person, or for important reasons of public interest of the EUEuropean Union or Member State);

d. the right to lodge a complaint to the President of the Office for Personal Data Protection, if you feel that the processing of your personal data violates the provisions of the GDPR.

9. You are not entitled to:

a. in connection with art. 17 clause 3 lit. b, d or e GDPR, the right to delete personal data;

b. the right to transfer personal data referred to in art. 20 RODO;

c. based on Article. 21 GDPR, the right to object to the processing of personal data, because the legal basis for the processing of your personal data is Art. 6 clause 1 lit. c GDPR.
