Cleaning service in buildings and communal areas and in the Complex of Municipal Market Halls in Gdynia managed by the Board of Buildings and Municipal Apartments in Gdynia, the description of the subject of the contract was divided into 10parts.

Main Site: Gdynia, districts Leśne Plots, Grabowek, detailed location in accordance with Annex 3 to the ToR.

Cleaning service in District No. 1 Plots Leśno, Grabowek service areas: buildings 627 m2, green areas: paved surface 7 190, 35m2; unconfirmed area 27 637m2

A detailed description of the subject of the order is included in Annex 3 and 4 to SIWZ.

Contractors who are not subject to exclusion and meet the conditions for participation in the procedure specified in Chapter VII of the ToR: the Ordering Party will exclude the Contractor from the contract award procedure in the circumstances indicated in art. 24 paragraph 1 point 12 23 of the PPL Act and provided for in art. 24 paragraph 5 points 1 and 8 of the PPL Act. The offer consists of: offer form, EAT form - preliminary statement confirming the absence of grounds for exclusion and compliance with the conditions for participation in the procedure, third party obligation - if applicable, power of attorney if applicable, proof of security deposit. Statements and documents to be submitted by the Contractor whose highest bid (procedure art. 24aa Public Procurement Law) at the request of the Employer in the designated territorywill pass, not less than 10 days: List of services rendered, and in the case of periodic services also performed within the last 3 years before the offer submission date; information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13-14; 21 of the Public Procurement Law, declaration of the Contractor about the lack of a final court judgment or final administrative decision regarding arrearswith the payment of taxes, fees or social security or health insurance contributions, the specimen declaration is attached as Annex 8 to the ToR; the Contractor`s declaration of no decision against him as a precautionary measure prohibiting him from applying for a public contract, a specimen of which is attached as Annex 8 to the ToR; a certificate from the competent tax office confirming that the contractor is not in arrearstax treatment; certificate of the Social Insurance Institution or the Agricultural Social Insurance Fund (Kasa Rolniczego Ubezpieczenia Społecznego) or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions; excerpt from the relevant register or from the central register and information on economic activity if separate provisions require entry in such register; statementcontractors not to pay taxes and local fees, referred to in the Act of 12 January 1991 on local taxes and fees (i.e. Dz. U of 2019 1170), whose template is Annex 8 to the ToR; information were included in chapter VII, VIII, XII of the ToR.
