The subject of the orders are worksmaintenance on the Biała Tarnowska River at km 88 150 88 450 in Brunara by filling a bank of 222 m. The scope of the robot includes: • laying the felling slab without affixing to the filled in.

• covering the edge of the edge with the natural foundry from the quarry (filling in the wild without laying) stone with D 80 cm

This condition will be met if the contractor showsthat: is insured against civil liability in the scope of conducted activities related to the subject of the order for a value not less than the amount of the bid submitted. The Employer provides the following grounds for exclusion: art. 24 section 5 items 1.2 and 8. Participation. 1. The Contractor who refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and fulfillment to them, in so far as it relies on their resources, the conditions for participation in the proceedings also includethese entities - the Contractor who intends to entrust the execution of a part of the orders to subcontractors, in order to demonstrate the lack of grounds for exclusion against them as regards subcontractors. - Principles and methods of completion EAT is available on the Employer's website at the place of placing the subject proceedings and is included in the explanations Public Procurement Office on-the Office's website, in the Knowledge Repository, in the Uniform European Order Documents tab.

2. In terms of "part IV of the qualification criteria" EAT, the Contractor may confine himself to completing the section, in which case the Contractor does not fill in any of the remaining sections in Part IV. EAT should be sent in an electronic form with a qualified electronic signature. statementsentities submitting jointly EATs and entities making the potential available on the EAT form should be in the form of an electronic document signed with a qualified electronic signature by each of them in the scope in which they confirm the circumstances referred to in art. 22 ust. 1 of the Public Procurement Law. The same requirement applies to EAT submitted by a subcontractor, pursuant to art. 25a para. 5 point 1 of the Public Procurement Law

4. The electronic communication means used for depositing EAT by the contractor is e-mail. ATTENTION! Placing an EAT along with an offer on a data carrier (eg CD, pendrive) is not allowed, because it does not constitute its submission using means of electronic communication within the meaning of the provisions of the Act of 18.7.2002 on the provision of electronic services.EAT should be sent to the following e-mail address: JEDZ-KR@wody.gov.pl. The ordering party allows in particular the following format of the data sent: .pdf, .doc, .docx, .rtf, .xps, .odt.

B) Contractor fills up the EAT by creating an electronic document. Can use

With ESPD tool or other available tools or software that enable you to fill in the EAT and create an electronic document, in detailin one of the above format.

Note: due to problems related to the correct verification of received documents EAT, please do not use Polish characters in the file names.

c) After creating or generating by the contractor an electronic document EAT, the contractor signs the above. document by a qualified electronic signature issued by a qualified trust service provider, beingit is an entity providing certification services - an electronic signature that meets the security requirements set out in the Act. d) The signed EID electronic document should be encrypted, i.e. provided with an access password. For this purpose, the contractor can use the tools offered by the software in which the document is prepared (eg Adobe Acrobat), or use the accesson the market open-source license tools (eg: AES Crypt, 7-Zip and Smart Sign) or commercial open-source tools.
