Serological reagents: standard blood cells for studying the ABO group layout: A1, B, 0 for the Laboratory of Transfusion Serology with the Blood Bank - item 1 (according to Part IX A SIWZ)

List of statements and documents confirming no grounds for exclusion:

1. Information from the National Criminal Record in zakreferred to in Article 24 sec. 1 points 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submitting bids.

2. Declaration of the contractor about the lack of a decision against him as a preventive measure for the prohibition of applying for a public order.

3. Declaration of the Contractor about the failure to issue a final court judgment or final administrative decision about zalegan against himand in the payment of taxes, social security or health insurance premiums, or in the event of such a verdict or decision, documents confirming the payment of such claims together with any interest or fines or a binding agreement regarding repayment of such claims. Current excerpt from the relevant register or from central records and information aboutbusiness activity, if separate regulations require an entry in the register or records, in order to confirm the lack of grounds for exclusion based on art. 24 sec. 5 point 1 of the Act.

5. The Contractor`s declaration of affiliation or lack of belonging to the same capital group, in the case of being part of the same capital group, the contractor may submit together with the declarationagreements or information confirming that connections with another contractor do not lead to distortion of competition in the proceedings.

Information on documents confirming that the offered supplies fulfill the requirements specified by the ordering party:

1) Declarations regarding possession:

A. declaration of conformity and CE marking or / and entry / registration in the register of medical devices and referencesotow responsible for their introduction and / or notification / notification to the President of the Office for Registration of Medical Devices, Medical Devices and Biocidal Products or data transfer form;

2) Documents in accordance with the requirements of the European Union for this group of reagents (marked with CE Mark together with the number of the Notified Body for reagents from Lists A and B);

3) Descriptions, i.e. e.g..: leaflets, catalogs, instructions, folders or other documents and information materials confirming unambiguously that the offered products meet the requirements for the description of the subject of the order specified in Part IX A of the ToR.

A. The Contracting Authority requires the Contractor to certify the authenticity of the documents (referred to in point 3), i.e. in the form of a written certificate - according to the Model: Annex no. No. 5 to SIWZ.
