For the purchase of these products, the necessity papers no. 68 / 23.1.2019, prepared by Dr. Moldovan Adriana and 2303 / 18.2.2019, prepared by Dr. Danciu Nicolae Danut, approved by the management of the unit.

Main Site: County HospitalThe minimum and maximum quantities for each product in the lot can be found in the tender specifications - Annex 1.

Tenderers, associate tenderers, supporting third parties and subcontractors must not find themselves in the prescribed situations to art. 164, 165, 167 of Law no. 98 / 2016.

The failure of DUAE along with the offer calls for rejection of the offeras unacceptable.

The manner by which the fulfillment of the requirement can be demonstrated: the economic operators participating in the award procedure (including any third parties and subcontractors) will be filled in with the information relevant to their situation and signed accordingly. The DUAE form is signed with the extended electronic signature.

The supporting documents proving the fulfillmentof the ones assumed by completing the DUAE are to be submitted, at the request of the contracting authority, only by the tenderer ranked first in the intermediate rankings drawn up at the completion of the evaluation of the tenders.

The supporting documents proving the fulfillment of the declarations in the DUAE can be: tax on outstanding debts related to tax paymentstaxes, or contributions to the consolidated general budget (local budget, state budget, etc.) valid at the moment of presentation, according to art. 165 of Law 98/2016, - Tax certificates regarding the lack of overdue debts related to tax payments, local taxes and other local budget revenues issued by the City Hall (Local Council) valid at the moment of presentation, according to art. 165 of Leg98/2016,

- The criminal record of the economic operator and all the members of the administrative, management or supervisory body of the respective economic operator or of those having the power of representation, decision or control within it, so as evidenced by the certifying certificate issued by the ONRC / act of incorporation valid at the time of the presentation,

- as the case may be, which demonstrates that the economic operator may benefit from the derogations provided for in Art. 166 par. (2), art. 167 par. (2), art. 171 of Law no. 98/2016 on public procurement, - other editorial documents, as the case may be.

The economic operator will fill in the corresponding requirement in the DUAE form in the awarding documentation.
