Delivery of medicinal products for UCK
Reference Number: 142 / PN / 2019
1. The subject of the order is the delivery of medicinal products for UCK in the quantity, assortment and requirements specified in Annex 3 to the ToR.
According to Annex 3 to the ToR - This condition will be fulfilled if the Contractor beforea permit to run a business covered by the subject of the order, i.e. a license for running a pharmaceutical wholesaler or permission for the co-production, if the contractor is a manufacturer or respectively for the contractor running the consignment warehouse permit to run a warehouse containing marketing authorization for medicinal products.on the basis of the completed Part IV: Eligibility criteria by submitting a general statement regarding all the eligibility criteria in point α (alpha) in the GOOD
Ordering does not set a specific condition in this respect.
In implementing the information obligation of the Administrator of personal data, we kindly inform you that:
1. The administrator of your personal data is the University CenterClinical, Independent Public Health Care Center based at ul. Dębinki 7, 80-952 Gdańsk, POLAND, registered in the 7th Economic Department of the National Court Register of the District Court Gdańsk-P-łnoc in Gdańsk under KRS number 0000122150, NIP 957-07-30-409, REGON 000288640.

3. Your personal data is processed (including collected) by UCK solely for the purpose of: a) conducting a public procurement procedure, b) seeking possible claims. Legal basis for processing your personal data by UCK: art. 6 par. 1 lit. c The RODO, according to which the processing of personal data is allowed if it is necessaryto fulfill the legal obligation of the administrator. Providing personal data by you is compulsory, it is a statutory requirement and it results from the Public Procurement Law. Failure to provide such data makes it impossible to carry out public procurement proceedings with you.
5. Your personal data may be transferred to the following categories of recipients:
a) entitym entitled to legal persons; - b) entities cooperating with UCK with regard to the implementation of services necessary for the implementation of the said process and management of our entity, in particular entities supplying UCK with technical and organizational solutions enabling them to be implemented, and in particular service providers: teleinformatic,
- accountants, - legal, advisory, orafrom supporting UCK in pursuing due claims - in the event of their occurrence (in particular law firms, debt collection companies), - courier and postal, - archiving, - related to the disposal of documentation and other media containing personal data; - c) persons authorized by the data administrator, including our employees and co-workers who must have accessp to the data to perform your duties; d) persons and / or entities authorized by you;
e) the founding body of the UCK (Medical University of Gdańsk) in connection with the exercise of corporate governance over UCK;
f) other persons and / or entities due to the openness of the proceedings.
6. Your personal data will be kept for the period required by law. Subject to thesermin, if the purpose of processing your data is to pursue claims, we process data for this purpose during the period of prescription of claims arising from the provisions of the Civil Code. After the above-mentioned periods, your data is deleted or subjected to anonymisation.
7. Your personal data will not be transferred to a third country or an international organization.
8. You have the rightaccess to the content of your data, rectification, requesting their removal or limiting their processing. You can also use the right to object to their processing and the right to transfer data to another data controller.
9. You have the right to file a complaint with the supervisory body (until 25/05/2012 GIODO, from 25/05/2018 PUODO), when you consider that theirThe law infringes the EU Regulation of the European Parliament and of the Council (EU) 2016/679 of 27.4.2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (so-called general regulation on the protection of personal data, at the outset of the RODO).
10. Your personal data is not subject to automated processingdecisions, including profiling.
