The subject of the order is the gradual delivery of food along with its distribution to the place of winter feeding of a herd of bisons in the Żednia Forest District as part of the implementationof the project "Comprehensive protection of the European bison in Poland" (contract for co-financing POIS.02.04.00-00-0027 / 18-00 of 30.5.2019 as part of Measure 2.4 Priority Axis II of the Operational Program Infrastructure and Environment 2014-2020).
Main Site: The place of performance of the contract will be the administrative area of the Żednia Forest District.
3. Description of the object of the order
3.1. Material scope of the object of the order
Winterfeeding of bison the supply of food along with its distribution in 2019-2020 in the Żednia Forest District
The subject of the order is the gradual delivery of food along with its distribution to the place of winter feeding of a herd of bison in the Żednia Forest District as part of the project implementation. "Comprehensive protection of the bison in Poland" (contract for co-financing POIS.02.04.00-00-0027 / 18-00 of 30.5.2019 as part of Measure 2.4 Priority Axis II of the Operational Program Infrastructure and Environment 2014-2020).
1) As part of the delivery, the contractor will deliver and unload in the indicated places in total:
a) 16 tonnes of silage;
b) 4.4 tonnes of oats ;
c) 10 tons of fodder beet.
Dates of individual deliveries and quantities will be agreed immediately before delivery with the designated employee of the ordering party.
2)The period of delivery is the period of winter feeding of bison falling:
a) 2019 year (2 months: November, December)
Amount of feed:
- silage, 8 tonnes,
- fodder beet, 5 tonnes,
- oats, 2.2 tonnes.
Schedule feed supply for 2019:
- silage [month / ton] November: 4; December: 4; total: 8,
- beetroot [month / ton] November: 2.5; December: 2.5; total: 5,
- oats [month / ton]November: 1.1; December: 1.1; total: 2.2.
b) 2020 (5 months: January, February, March, November, December)
Amount of food:
- silage 8 tons,
- fodder beet 5 tons,
- oats 2.2 tons.
Food delivery schedule for 2020:
- silage [month / ton] January: 1.6; February: 1.6; March: 1, 6; November: 1.6; December: 1.6; total: 8,
- beetroot [month / ton] January: 1; February: 1; March: 1; letterrainfall: 1; December: 1; total: 5,

3) Requirements for the object of delivery, i.e. forage:
a) silage of good quality silage from dried grass from the first swath;
b) crushed oats, healthy, without mechanical impurities with humidity up to 15%;
c) good quality fodder beet harvestedhall before frosts, without mechanical impurities and signs of rot.
4) Delivery rules:
a) unloading feed in Sokole forestry in branch 59b, or in the place indicated by the contracting authority depending on where the bison live, in January, February, March, November, December;
b) feed and unloading of feed will take place once or twice a week depending on the waweather conditions in the presence of an employee of the forest inspectorate.
5) As part of the delivery, the contractor will be required to clean up the feeding places, i.e.: cleaning of feeding places consisting of taking away food residues and its removal from the forest inspectorate's area;
b) final cleaning of the feeding place in which was given a juicy food at the end of the feeding period.
6) Proper implementation of fastingLow deliveries will be each time confirmed by a feed delivery / collection protocol (Annex 8 to the ToR) signed by both parties.

3.3. General rules for the implementation of the object of the contract:
1) placem of the contract will be the administrative area of the Żednia Forest District;
2) The Contractor may entrust the implementation of elements (parts) of the subject of the contract to subcontractors. In the event of the intention to perform the subject of the contract with the participation of subcontractors, the contractor is required to indicate in its offer part of the contract (material scope) which it intends to entrust to subcontractors andsubmission of subcontractors by the contractor. This indication should be included in the offer. In the absence of an indication in the subcontracting offer, the contractor will be able to introduce the subcontractor only under the conditions specified in the contract.in the circumstances referred to in art. 24 paragraph 1 point 12-23 Public Procurement Law, art. 24 paragraph 5 points 1-2 and 4-8 of the Public Procurement Law
If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland instead of the documents referred to in section VI.3 point I of the notice
1) referred to in section VI.3 point 1 of the notice, 2, 3 submits the document (s) issued in the country in which he has his registered office or place of residence, confirming the answerediting that:
a) its liquidation has not been opened or bankruptcy has been announced (issued not earlier than 6 months before the deadline for submission of bids);
b) it is not in arrears with the payment of taxes, fees, social security contributions. or health insurance or that he has entered into an agreement with the competent authority regarding the repayment of these claims together with any interest or fines, in particular he has obtainedby law, release, deferment or installment of overdue payments or suspension in whole of the execution of the decision of the competent authority (issued not earlier than 3 months before the deadline for submission of bids);
2) referred to in section VI.3 of the notice, point 4 shall submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent authority pcourt or administrative of the country in which the contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the PPL and in art. 24 paragraph 5 points 5 and 6 (issued not earlier than 6 months before the deadline for submission of bids).
If in the country in which the contractor has its registered office or place of residencethe person or place of residence has the person to whom the document relates, the documents referred to in item 2) do not appear, they are replaced by a document containing respectively the contractor's statement, indicating the person or persons authorized to represent it, or the statement of the person to whom the document was intended, submitted to a notary public or a judicial, administrative or self-governing bodyprofessional or economic government competent for the registered office or place of residence of the contractor or the place of residence of that person. referred to in item 2), to the extent specified in art. 24 paragraph 1 point 14 and 21 of PPL and usVol. 5 point 6 of the Public Procurement Law If in the country in which the person to whom the document was supposed to reside does not seem such documents, it is replaced by a document containing the person's statement submitted before a notary public or before a judicial, administrative or professional or economic self-government body competent for the place that person's residence (issued not earlierless than 6 months before the deadline for submission of bids).
The Purchaser does not impose any requirements in this respect.
To confirm the absence of grounds for exclusion from art. 24 paragraph 1 point 23 of the Pzp, a declaration of belonging or not belonging to the same capital group shall be submitted within 3 days from the date of posting on the website the information specified in art. 86 paragraph 5 of the Public Procurement Law Together with PLNrequesting a declaration, you can provide documents or information confirming that links with another contractor do not lead to a distortion of competition. In the case of jointly applying, each of the contractors submits a declaration.
The call consists of:
1. an excerpt from the relevant register or from CEIDG, if separate provisions require an entry in the register or register;
2. certificate of the competent directorTax Office confirming that the contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids or another document confirming that the contractor has entered into an agreement with the relevant tax authority in repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferral or distribution provided for by lawinstallments of late payments or withholding in full of the decision of the competent authority;
3. certificate of the relevant local organizational unit of ZUS or KRUS or other document confirming that he is not in arrears with payment of social security contributions or health issues issued not earlier than 3 months before the deadline for submitting bids or other document confirming that he has made an agreementnot with the competent authority regarding the repayment of these claims together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the execution of the decision of the competent authority;
4. information from the KRK in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the PPL and regarding conviction for an offense are a penaltyart, to the extent specified pursuant to art. 24 paragraph 5 points 5 and 6 Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids;
5. statement that he has not issued a final court judgment or final admission decision on late payment of taxes, fees or social security contributions or health, or in the event of such a judgment or decision being issued, documentsdeclaring payment of these debts together with any interest or fines, or concluding a binding agreement on the repayment of those debts;
6. a statement that he has not been awarded a decision as a precautionary measure forbidding public procurement in relation to the condition of exclusion in art. 24 paragraph 1 point 22 of the Pzp;
7. statement on the lack of issue of a final productto the convicting court for an offense of restriction of liberty or a fine to the extent specified by the contracting authority pursuant to art. 24 paragraph 5 points 5 and 6 of the Pzp;
8. statement that no final admission decision has been issued on breach of obligations arising from provisions of labor law, environmental protection or social security provisions. to the extent specified pursuant to art. 24 paragraph 5 point 7 of the Pzp;
9.declaration of non-payment of local taxes and fees, referred to in the Act of 12.1.1991 on local taxes and fees.
The offer must include:
1. offer form;
2. power of attorney: to submit an offer, for a proxy appointed by joint applicants for the contract;
3. entity 3 obligation or other document serving to demonstrate potential access to the contractoru;
4. EAT as the contractor's own statement;
5. EAT for each of the entities on which the contractor relies;
6. EAT for each of the contractors jointly applying for the contract;
7. bid bond in non-cash form in the original in post. electron. Wound. kwal. set fire. electron.

In sesection II.2.5 - the word 'quality' is deleted. The criteria for the evaluation of the bids are: price - 60%, payment term - 40%.
