1) application letter model DC1 (latest version) or equivalent;

2) candidate declaration modele DC2 (last version) or equivalent (https://www.economie.gouv.fr/daj/formulaires-marches-publics);3) mandate of the insurance company to the broker or agent if the case;

4) power of the person authorized to hire the candidate;

5) insurance certificate, ORIAS and financial guarantee compliant with the insurance code (broker agents);

6) if form DC1 is not used, thestatement of honor that the candidate does not enter into any of the bidding cases provided for in art. 45 and 48 of the ordinance 2015-899 of 23.7.2015; 7) pursuant to art. 49 of the decree n 2016-360 of 25.3.2016, the candidate can submit his application in the form of a Single European Market Document (DUME) instead of the documents mentioned in art. 48 of the decree n 2016-360 du 25.3.2016.
