ANALYSIS OF THE PRIMARY HEALTHCARE SERVICES IN HAITI

A. INVOLVED ORGANIZATIONS, PROJECT BACKGROUND AND OBJECTIVES
Involved organizations:
The International Finance Corporation (IFC) is the largest global development institution that is focused exclusively on the private sector. As part of the World Bank Group, IFC-s vision is that people should have the opportunity to escape poverty and improve their lives by engaging in economic activity. To achieve this goal, IFC finances private sector investments, mobilizes capital in international financial markets, facilitates trade, helps clients improve social and environmental sustainability, and provides advisory services to businesses and governments.
Background:
Haiti is the poorest country in the Western Hemisphere, with a Gross Domestic Product (GDP) per capita of $756 in 2019 and a Human Development Index ranking of 169 out of 189 countries in 2019. According to the Human Capital Index, a child born today in Haiti will be only 45% as productive as an adult that enjoyed full education and health. Political instability has hindered Haiti-s economic and social development. GDP is estimated to have contracted by 0.9% in 2019, and the country has experienced large currency volatility, (24% of depreciation between January and August 2020, and 83% of appreciation in the month of September ) and rampant inflation (25.7% in July 2020 ). The economic outlook is fraught with downside risks amid the COVID-19 pandemic and an unresolved political crisis .
Haiti-s health outcomes are poor, even compared to other low-income countries, and depend on wealth levels. Both infant and child mortality remain high, the probability of dying before the age of five is 6.5%. Life expectancy at birth is 62 years for men, and 66 years for women, well below a global average of 73 years. Potential explanations include, among others, low service coverage in some regions, lack of practicing healthcare professionals, poor quality of available health services, high user fees, and very low levels of utilization of services. Indeed, utilization rates for outpatient services are only around 0.5 visits per person per year which compares to 1.9 visits per capital in Colombia, 2.3 visits in Costa Rica, and 2.8 visits in Mexico .
Access to primary care of good quality is very limited in Haiti. Only 23% of the population live within 5km of a facility with service delivery of good quality and only 46% of the facilities include maternity beds. Multiple reports suggest that women illegally cross into the Dominican Republic to give birth there, quoting high costs of private medical services in Haiti and low coverage of affordable services . Moreover, there is a concentration of primary care facilities only in and around Port-au-Prince . Ouanaminthe, located in the North of Haiti, 250km away from Port-au-Prince and on the border with the Dominican Republic, has limited public healthcare services available, with only one public hospital with a deteriorated infrastructure.
Objective:
IFC is seeking to hire a Firm to conduct a study of the healthcare market in Haiti that will form the basis for a feasibility analysis of a new primary health care facility and potentially a new hospital in Ouanaminthe. The regional scope of this study is Haiti, with a focus on Ouanaminthe.

B. SCOPE OF WORK
IFC is seeking to hire a Firm to perform five main activities. Unless otherwise specified, the report should cover Haiti, and disaggregate data by region. Where specified, a focus lies on the Ouanaminthe region. IFC-s project will primarily target low- and medium-income households which should be the focus of this assessment. To ensure business model viability, a service offering that includes high-income households, or households benefiting from remittances, may be considered. Historical and expected figures should be provided, whenever possible, according to guidance below:
1. Market overview
a. Briefly describe the primary healthcare market in Haiti;
b. Estimate the demand for healthcare services, including
i. Current and historical numbers (last five years) of diseases that affect the population, with a breakdown by age and gender
ii. Current and historical numbers (last five years) of healthcare service needs in Haiti by region, with a breakdown by age and gender, and healthcare services frequently sourced abroad or in different regions (e.g. women giving birth in the Dominican Republic, influx of patients to a certain region to treat specific diseases)
iii. Current and historical numbers (last five years) about the use of formal and informal healthcare services in Haiti, by region
iv. Expected evolution of demand for healthcare services in the next 5 to 10 years by region, including the size of the potential market for primary care (i.e. number of potential walk-in patients).
