Description : Annual PE, to iniclude the following tests:

1. Routine Urinalysis

2. Complete Blood Count

3. Chest X-Ray (Plain): Posterior-Anterior (PA) View

4. 12-L Electrocardiogram (ECG)

5. Blood Chemistry, to iniclude: Fasting Blood Sugar, SGPT, Lipid Profile (Triglycerides, HDL, LDL, VLDL), Blood Urea Nitrogen (BUN), Creatinine, Uric Acid

6. HBA1c, for (a) employees diagnsed with diabetes, (b) employees identified as having increased risk of type 2 diabetes, and (c) employees with symptoms of diabetes

7. Breast Mammogram for females 40 years old and above

8. Pap Smear for females 35 years old and above and married females regardless of age

9. Prostate Specific Antigen (PSA) for males 40 y/o and above
