The Ordering Party invites you to participate in the procedure in the basic mode in variant 2 for: "Comprehensive provision of laundry services for the needs of the Institute of Mathematics of the Polish Academy of Sciences, divided into tasks: Task 1: Provision of laundry services for the needs of the Center IMPAN Research and Conference Center in Będlewo. Task 2: Providing laundry services for the needs of the Institute of Mathematics of the Polish Academy of Sciences in Warsaw "based on Article 275 point 2 of the Public Procurement Law. The conditions for participation in the procedure and submission of offers are specified in the SPECIFICATION OF ORDER CONDITIONS (SWZ) placed in the Attachments tab. ATTENTION! 1. The offer is prepared, under pain of invalidity, in electronic form. 2. The Contractor is obliged to submit via the Purchasing Platform in the "Attachments" tab, signed by authorized persons, the following documents and declarations: 1) Offer in accordance with the content of the Offer Form, constituting Annex No. 3 to the SWZ and the Price Form appropriate for a given Task (part), constituting Annexes No. 4 a-c to the SWZ; 2) Declarations constituting Annexes No. 5-7 to the SWZ3) Power of attorney/powers of attorney on the terms described in the SWZ in a form consistent with the regulation on the method of preparing and transmitting information (if applicable);4) Declaration regarding the division of responsibilities between contractors, in accordance with point 11.3.
