Abstract: This procurement will identify a Bidder who will deliver a Community Wellbeing Service.This proactive prevention provision will aim to meet the needs of adults who are at risk of and/or who have multiple long-term conditions/health and care needs and who may not be engaging with other health services.The service will commence 1st October 2024 for 3 years with the optional extension period of 2 x 24 months. Maximum budget £900, 000 per annum, £6, 300, 000 for the total 7 years length.The opportunity to bid is open until 4th April 2024 at 14:00 hours.The HIOW ICB Strategy lays out five main priorities, one of which is the focus on 'Good health and proactive care.'
