Collection, transport and disposal of medical waste The subject of the order is: waste collection, transport and disposal service with code 18 01 04. The Ordering Party provides for the possibility of awarding supplementary orders pursuant to Art. 214 section 1 point 7 - up to 30 % of the value of the basic order. List of declarations submitted with the offer: 1) Declaration of meeting the conditions for participation in the proceedings and demonstrating the lack of grounds for exclusion from the proceedings, prepared in the form of the Single European Document Orders (EAT) - according to the template constituting Appendix No. 2 to the SWZ. The Ordering Party informs that the Contractor may also use the tool available at: https://espd.uzp.gov.pl/ when completing the declaration on the JEDZ form. a)The declaration states preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the procedure as at the date of submission of offers. b) The declaration constitutes a preliminary confirmation that the entity providing the resources is not subject to exclusion and meets the requirements conditions of participation in the procedure, to the extent to which the contractor refers to its resources as at the date of submission of offers c) The declaration is submitted by each of the contractors jointly applying for the contract.
