As part of the feasibility study commissioned by the Vienna State Target Management, the use of telemedicine measures for heart failure patients is to be tested in a pilot phase As part of the implementation phase, a telemonitoring model will be established to care for this patient group. A Tele-Care Center (TCC) was set up for this purpose.
