1. The subject of the contract is to conduct laboratory tests for project participants: Health support of employees of "Busko-Zdroj Uzdrowiska" S.A. employed in "Uzdrowisko Busko-Zdroj" S.A. including:

a) 183 Blood testing packages expanded within which: hepatic tests (ALT, AST, ALP, ALP, Bil, GGTP), morphology, iron, urea, ob., Creatinine, uric acid, electrolytes (na, k), lipidogram (chol, HDL, Nie-HDL, LDL, TG), glucose, marker breast and/or ovarian cancer-(BRCA1, BRCA2, PALB2) marker 19-9, marker of colorectal cancer (CEA), Marker Rak stomach antigen CA 72-4, thyroid package (TSH, FT4, P/c. P. TSH receptors (TRAB), FT3, Anti-TG, Anti-TPO)-transport of the collected material to the laboratory, analysis of the collected material in the laboratory, providing all necessary materials necessary to collect the material, among others vials, gloves, needles, disinfecting fluids;

b) 28 Diagnostic packets of prostate cancer - prostate cancer panel: CHEK2 (1100DELC, IVS2+1G A, DEL5395, I157T), NBS1 (NBN) 657DEL5, HOxB13 (G84E), RS188140481 A/T and PSA panel designation (dog, FPS, FPS/PSA indicator) - transport of the collected material for laboratory, Analysis of the collected material in the laboratory, providing all necessary materials necessary to collect the material, including vials, gloves, needles, disinfecting fluids;
