1. The Bangko Sentral ng Pilipinas requests quotations for the hereunder requirement:

Brief Description of the Requirement : Calibration Services for the following Work Environment Measurement (WEM) Instruments (12 lots), as per BSP Technical Specifications, Terms of Reference and Price Breakdown:

1. 1 lot Calibration Services for Heat Stress Meter

2. 1 lot Calibration Services for Laser Distance Meter

3. 1 lot Calibration Services for Lux Meter

4. 1 lot Calibration Services for pH Meter

5. 1 lot Calibration Services for Sound Level Meter

6. 1 lot Calibration Services for Ultrasonic Thickness Gauge

7. 1 lot Calibration Services for Thermo-Anemometer + IR Thermometer

8. 1 lot Calibration Services for Sound Level Calibrator

9. 1 lot Calibration Services for Airflow Calibrator

10. 1 lot Calibration Services for Portable Multi-Gas Monitor

Completion Period : Within sixty (60) calendar days reckoned from the date stipulated in the Notice to Proceed to be issued by Sectoral Operations Management Department-Currency Management Sector

Warranty Security : Five percent (5%) of the total contract price valid for three (3) months from issuance of Certificate of Acceptance

2. All quotations/proposals that meet the deadline shall be opened by the SOMD-CMS Committee on Procurement at 2:30 P.M. on 04 June 2019. Late submission of quotations/proposals shall be considered as disqualified.



3. The Supplier with the Single or Lowest Calculated Quotation shall submit the following documents, within five (5) calendar days upon receipt of notice. Failure to do so shall result in the post-disqualification of its quotation/proposal:



A. LEGAL DOCUMENTS

a. Valid Y2019 Mayor-s/Business Permit, or the equivalent document for Exclusive Economic Zones or Areas.

b. Philippine Government Electronic Procurement System (PhilGEPS) Registration Number/Certificate.





B. TECHNICAL REQUIREMENTS

a. BSP Terms and Conditions, duly signed by the Supplier-s authorized representative on the Conforme page (1 page).

b. BSP Technical Specifications, duly signed by the Supplier-s authorized representative (1 page).

c. BSP Terms of Reference and Annex A, duly signed on each and every page by the Supplier-s authorized representative (3 pages).

d. Omnibus Sworn Statement.



Note: Use BSP prescribed form. Omission of any of the provisions in the Omnibus Statement shall be considered as post-disqualified.



4. The BSP assumes no responsibility whatsoever to compensate or indemnify any supplier for expenses incurred in the preparation of quotations/proposals.
