***amendment # 1 dated 7/12/19, is to add the additional requirements for new haven. They are listed on the pricing matrix clins 48-73. All vendors that have submitted a quote shall submit a revised quote to include the amendment requirements to be considered for award. Additionally all vendors shall submit with their quote the completed pricing matrix, please make sure you download and utilize the amendment # 1 matrix. This amendment removes the need to complete and submit page 4 of the rfq document. The vendor only needs to submit their pricing quote on their letter head and complete and submit the pricing matrix amendment # 1 dated 7/12/19. The due date for this rfq has also been pushed back based on this amendment. The new due date and time for quotes is 7/22/19 at 8:00 am est. This amendment changes the date listed on the rfq document and the date listed on the fbo posting.

Vendor shall submit-

1) quote on their letter head for all items listed on the pricing matrix 1 dated 7/12/19.

2) submit the completed pricing matrix 1 dated 7/12/19 to be considered for award/technically acceptable.

All items shall be brand name per the brand name justification.***
