This service includes the measurement of key metrics to inform a risk calculation of Cardiovascular disease for patients. Those assessed to be at risk are then placed on appropriate management plans.

Abstract: The Council hereby advertises an Authorisation Scheme for appropriately registered organisations, which would have access to patients registered with GP practices, for the provision of the NHS Health Checks scheme. This service specifically includes the measurement of appropriate metrics to support the assessment of likelihood of Cardiovascular disease, and if required following a check, the placement of patients on disease registers.Organisations wishing to provide the services under the Authorisation Scheme should apply by downloading the forms and other key documents from Source Leicestershire (http://www.sourceleicestershire.co.uk/contracts/show/id/17479).Please ensure (if not done so already) to register with Source Leicestershire in order to download the application form and other key documents.The Council reserves the right to terminate the authorisation scheme at any point should it deem this necessary.
