Tender Details

1. The subject of the order is the supply of endoprostheses, implants, titanium plates, screws and microscrews for the needs of the Otolaryngology Department with the Maxillofacial Surgery Subunit of the Provincial Integrated Hospital in Elblg, the detailed description, range and quantities of which are specified in Appendix No. 1 to SWZ.2. The Contractor will provide the Ordering Party with reusable instruments necessary for the implantation of implants, along with containers for their storage and sterilization, for the duration of the contract. (requirements and the scope of the equipment used are described in Annex No. 1 to the SWZ). The equipment will be loaned on terms and conditions specified in the lending agreement, the template of which is Annex No. 5 to SWZ.3. The Contractor will provide training for medical staff (5 doctors and 2 nurses selected by the Ordering Party) of the Otolaryngology Department with the Oral and Maxillofacial Surgery Subunit in the form of a practical course or by participating in in lectures on topics related to the ordered products.
