24/7 provision of medical services - KRP VI 1. The subject of the order is the provision of medical services for persons detained / brought by the Police to sober up and to be placed in a police room for persons detained for the Warsaw District Police Headquarters VI. 2. A detailed description of the subject of the order is included in Chapter XIX of the SWZ and Annex No. 4 to the SWZ. 3. Employment requirements based on an employment relationship in the circumstances referred to in Art. 95 Act: 1) The Ordering Party requires the Contractor or Subcontractor to employ, under an employment relationship, persons who perform the following activities within the scope of the order: administrative activities, including patient registration. 2) in progress execution of the order, the Ordering Party is entitled to perform inspection activities towards the Contractor regarding the compliance by the Contractor or Subcontractor with the requirement of employing persons performing the activities indicated in point 1 on the basis of an employment relationship. verification of compliance with these requirements, the Ordering Party is entitled in particular to request: a) declarations of the employed employee, b) declarations of the contractor or subcontractor about employing an employee under an employment contract, c) certified for compliance with an original copy of the employment contract of the employed employee, d) other documents − containing information, including personal data, necessary to verify employment under an employment contract, in particular...
