AGREEMENT for Comprehensive Coverage MAINTENANCE (OM)
01. SECTION 1 - GENERAL
01.1. SCHEDULE OF EQUIPMENT:
Supplier proposes to furnish OM MAINTENANCE on the following described equipment (“ELEVATOR OR EQUIPMENT”) TWO (2) UNITS “OTIS” ELEVATOR /// MACHINE No.I8SE0257D-C
01.2. LOCATION OF EQUIPMENT:
DAP BUILDING, San Miguel Avenue, Ortigas Center, Pasig City
01.3. TERMS OF AGREEMENT:
This service shall commence on the 30th day of July, 2026 and shall continue for an initial duration of one year and may be renewed upon mutual agreement of the parties.
01.4. MAINTENANCE FEE:
01.4.1. In consideration of the company providing maintenance services in accordance with the agreement, DAP agrees to pay the maintenance fee;
