Expression of Interest are invited for Wellness & Chronic Disease Management Programme

Terms & Conditions of the EOI
Expressions of Interest submissions should be accompanied by the following:
a. Copy of Certificate of Incorporation, Registration of Business or document indicating legal status of the entity.
b. A copy of a joint venture agreement if there is collaboration of entities.
c. Business profile of the entity.
d. Copy of register of Shareholders and Directors` information (including certified copies of identification documents).
e. Certificate of valid and industry relevant trading licenses.
f. Evidence of accreditation with relevant industry professional councils.
g. Evidence of technical capacity to provide the services required.
h. Proven record of previous related assignment - submit at least two (2) written references.
i. Evidence of financial capacity to undertake the service required.
j. Tax registration certificate, tax clearance certificate, proof of VAT registration or Tax Exemption letter/certificate where applicable
