Title : Voluntary Wellness Screening

Description : Contractor will provide all personnel, labor, tools, equipment, materials, permits, taxes, and licenses necessary for conducting Voluntary Wellness Screening services, and to provide confidential, onsite wellness screenings and follow-up guidance for eligible CAL FIRE Safety employees at, but not limited to: Sacramento, Ione, Riverside and 21 other units located throughout the State of California. The number of onsite testing locations will be provided based on needs assessment. The testing will operate on a three (3) year rotation cycle, traveling to specific Units each year, depending on each Unit¿s staffing pattern. The goals is to provide services to 1, 550 employees per year. All testing equipment will be provided and transported by the Contractor to all CAL FIRE units, statewide
