REQUEST FOR QUOTATION

SMALL VALUE PROCUREMENT

INSTRUCTION:

1. Bidders are required to read the instructions and fill all the blanks properly, particularly on the column under the offered brand. Non indication of the brand shall be construed as offering the specification reflected in the RFO.

2. The bidder-s/suppliers may submit alternate offer provided the BPI-SPAL-Baguio official canvass form shall be used (with the offered brand, unit price and total price), signed properly and attached together with the bidders/suppliers own canvass form. Please write legibly, non-compliance of which shall result to the automatic disqualification of proposal.

3. Specification herein provided are the minimum requirements of BPI-SPAL. Hence bidders are requested not to offer lower specification than required.

4. Quotations must include all kinds of taxes for the items/services listed hereunder, including delivery charges.

5. Price quotations submitted shall be valid for a period of THIRTY (30) days reckoned from the deadline for the submission of quotations as reflected in the PhilGEPS.

6. Terms of payment is within thirty (30) days from delivery of items.

7. Quotations/Proposals shall be submitted by the Procurement Coordinator/ Supplier to the Office of the BAC Secretariat at the BPI-BNCRDPSC, Baguio City.

8. Only sealed canvasses shall be considered by the BAC for ABC-s above P50, 000.00 and open canvass for P50, 000.00 and below

9. For sealed canvass, the bidder is required to affix his/her signature over printed name across the flap of the sealed envelope, non compliance of which shall result to the automatic disqualification of the proposal.

10. Bids in excess of the ABC shall be automatically rejected at bid opening.

11. Awarding shall be done per: PER LUMP SUM

12. DEADLINE FOR SUBMISSION OF BIDS :___July 15, 2019

13. Delivery Period/ Effectivity Period: Fifteen (15) working days upon receipt of P.O.

14. Availability of stock: 30 days reckoned from instruction no. 12

15. Mandatory Requirements: (Certified True copies of Valid Mayor-s Permit and BIR Certificate of Registration, Calibration certificate, Philgeps Registration). The proposal shall be accompanied by the aforementioned documents, otherwise, non-submission of which shall result to automatic disqualification of proposal.

16. The bidders shall reflect on the envelope the Purchase Request No. (PR No.), Reference No., the BAC shall not be responsible on the pre-emptive/premature opening of the proposal.
