1. Interested suppliers/bidders are required to read instuctions carefully.
2. Please quote your price(s) INCLUSIVE OF VAT opposite the article(s)/item(s) as per specifications.
3. Quotations shall be submitted to the BAC Secretariat at the Mayor's Office- Ms. Sharon C. Saingan, Ms. Arlene D. Bangit
Ms, Vennie A. Gallotan, Ms. Regie M. CariA+/-o, Municipal Assessor's Office-Mr. Gaston Arnel Jr. O. Dulnuan.
4. Interested suppliers/bidders are required to submit: PhilGEPS Registration Number, Mayor's/Business Permit and DTI Registration
or BIR Certificate, Income/Business Tax Return and Omnibus Sworn Statement upon receipt of Purchase Order (PO).
5. Any specification/s other than what is stated in the form will not be considered for evaluation.
6. Quotations must specify all kinds of applicable taxes including delivery and other fees to be charged.
7. Indicate brand names and model of the goods offered.
8. Price Quotations shall be valid THIRTY (30) days from the declaration of the winning bidder.
9. Mode of payment is through check, so kindly indicate name of account where payment will be made.
10. Submission of sealed quotations is encouraged.
11. Availability of stocks shall be for SIXTY (60) calendar days from the date of submission of quotation.
12. DELIVERY PERIOD shall be TEN (10) working days upon receipt of approved Purchase Order.
13. DATE & TIME OF SUBMISSION OF BIDS: August 8, 2018 @ 9:00 AM
We may purchase the item/s from your Company if the offer is found to be reasonable and advantageous to the Municipality,
but we reserve the right to accept or reject the offer or all quotations found not in order.
