1. Subject of the orderThe subject of the order is the provision of transport and storage services for bodies of persons (patients) from the departments or rooms indicated by the Ordering Party from both locations in Gliwice: - ul. Kociuszki 29, - ul. Zygmunta Starego 20, - ul. Kozielska 8, - ul. Radiowa 2, to the place where they are stored (Contractor's walkway) or to the Department of Forensic Medicine at the Oncology Center in Gliwice, ul. Wybrzee Armii Krajowej 15. Services, hereinafter referred to as "services" or "transport" or "subject of the order", will be performed in accordance with the conditions specified in the Specification of Terms of Order (SWZ), the Contractor's offer shall constitute annex to the contract and this OPZ.2. Scope of the orderThe order includes in particular: a. Transport of bodies from departments or premises indicated by the Ordering Party in locations in Gliwice at ul. Kociuszki 29 / ul. Zygmunta Starego 20 / ul. Radiowa 2 / ul. Kozielska 8 (ZOL) to the place where they are stored (Contractor's sidewalk) - estimated 1, 700 services over the entire duration of the contract. b.
