Tenders are invited for Supply of TB Health Consumables.

QUALIFICATION REQUIREMENTS FROM INTERESTED PARTIES:
• Evidence of Registration with Corporate Affairs Commission (CAC) with inclusion of Certificate of Incorporation and Articles of Association.
• Company profile and organizational structure with full name of organization and contact person, postal address, telephone/fax numbers, and e-mail addresses.
• Evidence of valid and current registration with Lagos Stale Public Procurement Agency.
• Evidence of company tax clearance for foe most recent /last 3 years (2015-2017 or 2016-2018)
• Audited financial statements of the company for the years 2015, 2016 and 2017
• Must be a manufacturer, major distributer or representative of the manufacturer/distributor of such consumable items
• Must have executed such supplies with either private, non¬governmental or governmental organization(s) in the last 4 years Evidence of such contract execution minimum of three with references.

ADDITIONAL INFORMATION:
Please note that the submission of these documents in response to this invitation shall not entitle any of the interested parties any claims against LASG/ LSMOH/ Global Fund.

All costs incurred by respondents as a result of this invitation and any subsequent requests for information shall be for the respondents account only.

Samples of items to be supplied may be requested for sighting from recommended contractors before contract award.
