NOTES:

1. Quotation may be submitted either typewritten (preferred) or handwritten.

2. ALTERNATIVE BIDS/A BID WITH OPTIONS SHALL BE REJECTED.

3. Request For Quotation can be obtained from the Office of Procurement Division on September 19 to 23, 2019.

4. Warranty shall be for a period of three (3) months, which shall commence after the final acceptance of the end-user.

5. Price validity shall be for a period of 120 calendar days from opening of bid.

6. Submit sample/s and brochure/s, if applicable, showing specifications of product being offered. Likewise state the brand in the RFQ or bid proposal.

7. If awarded, the bidder shall submit the required Registration Documents.

SCOPE OF WORK:

1. Mobilization.

2. Shut off power supply and apply LOTO.

3. Pull-out defective parts.

4. Install new compressor motor and IPCU board.

5. Pressure test the system and check for leaks.

6. Vacuuming of the system.

7. Charging of refrigerant.

8. Start-up the unit.

9. Testing and commissioning, submit operating parameters.

Notes:

1. Prospective contractors are required to conduct site inspection to check the models of existing compressor motor and IPCU board.

2. Only interested contractors that complied with our request to conduct site inspection are eligible to bid to ensure realistic bid proposal and the

supply of compatible replacement parts.

3. Participating bidders are required to specify the model/part number of the parts to be supplied.

4. Submit certificate of OEM of the supplied parts from Concepcion-Carrier Air Conditioning Company (CCAC), the Authorized Distributor of Toshiba

Air Conditioner in the Philippines to ensure the parts are new and genuine.

5. The contractor shall be an authorized product installer and service company of CCAC to have the expertise of a legitimate air conditioning company

for technical support and prompt and reliable after-sales service.

6. Refrigerant (R-410A) to be supplied by the end-user.
