The Rizal Medical Center (Rizal Med) through its Bids and Awards Committee-A (BAC-A), hereby invites all interested suppliers or distributors registered with the Philippine Government Electronic Procurement System (PhilGEPS), to submit their quotation on the item listed below, subject to the General Conditions stated herein, and submit not later than 16 April 2019.

I. TERMS OF PAYMENT



a. Payment shall be based on actual deliveries made by the supplier.



b. Pursuant to Circular Letter No. 2013-16 of Department of Budget and Management dated December 23, 2013, the Supplier/Contractor is encouraged to open an account with the Land Bank of the Philippines (LBP), the Government Servicing Bank of the Procuring Entity, to facilitate settlement of accounts payables due to the Supplier/Contractor. All charges incurred for payment through bank transfer with other banks shall be for the account of the Supplier/Contractor.





II. APPROVED BUDGET FOR THE CONTRACT



The approved budget for the contract is Sixty Nine Thousand Two Hundred Fifty Pesos (PhP69, 250.00)





III. GENERAL REQUIREMENTS:



1. PhilGEPS Registration Certificate, Mayor-s/Business Permit, Income/ Business Tax Return, Omnibus Sworn Statement, Certificate of Product Registration (CPR), Certificate of Analysis (COA), Certificate of Good Manufacturing Practice (CGMP) and Certificate of Distributorship (COD) shall be attached upon submission of the Quotation.



2. All quotations shall be considered as fixed price, and not subject to price escalation during contract implementation.



3. Bidders are entitled to one (1) bid only, otherwise all bids made shall automatically be rejected.

The Rizal Med assumes no responsibility whatsoever to compensate or indemnify Bidders for any expenses incurred in the preparation of their Quotation/Bid.
