Please quote your lowest price on the item/s listed below, subject to the General Conditions,

stating the shortest time of delivery and submit your quotation duly signed by your authorized

representative.



NOTE:

1) The approved budget for the contract is a‚+/-135, 000.00

2) All entries must be filled up as much as possible. .

3) Service/Delivery period is within seven (7) calendar days.

4) Warranty shall be for a period of three (3) months for supplies and one (1) year for equipment

from date of acceptance by the procuring entity.

5) Price validity shall be for a period of _______ calendar days.

6) PHILGEPS registration certificate shall be attached upon submission of the quotation.

7) Bidders shall submit original brochures showing certifications of the product being offered.

8) The bidder shall give sample/s of the item/s quoted.

Must be approved by the National Institute for Occupational Safety and

Health (NIOSH) against chlorine gas

The Full Facepiece must be made of silicone/Thermoplastic Elastomer (TPE)

4-point Harness Type, Bayonet Connection type and must have a valve that

makes breathing easier and provide cool-dry comfort.

Filter Cartridge can be use up to 50 times the Permissible Exposure Limit

to Chlorine gas

Terms and condition:

1. Participating bidders shall submit readable brochures with

techinical data subject for evaluation and acceptance of the end user.
