Bidders are required to read the instructions:

1. Any specifications other than those required/stated to this form shall not be considered in the evaluation of bid.

2. Quotation/s must include all kinds of taxes for the item(s) services are stated hereunder, including delivery charges.

3. Interested Bidder shall obtain Request for Quotation (RFQ) Form thru fax or thru to BAC Secretariat of BFAR-3 City of San Fernando, Pampanga.

4. Always indicates the brand/model or a brochure of the offered item(s) or product for evaluation.

5. PhilGEPS Registration number shall be submitted prior to the awarding of contract for procurement above P50, 000.00

6. Quotation(s) proposal(s) shall be submitted to the office of the BAC Secretariat, BFAR-R3, Government Center, Bgy. Maimpis, City of San Fernando, Pampanga

7. Only sealed canvass shall be accepted. The bidder is required to affix his/her signature, across the flap of the sealed envelope.

8. Awarding shall be done by lot/package.

9. All representative must bring Authorization Letter and signed by Manager or owner of company their represent.

10. All documents submitted must certified true copy of issuing Agency and bring the original copy for verification.

11. All interested bidder/supplier must submit updated Company Profile and PhilGEPS Registration Certificate (Original) for verification.

12. Deadline for submission of bids is on or before 2:00 PM of November 25, 2019

13. The BFAR-RO III - BAC reserves the right to reject any or all bid, waive any formality therein without disclosing the reason or basis and accept only the bids it deem is most advantageous to the government.
