The subject of the order is the successive delivery of materials used for eye microsurgery to the Associate 6700 and EVA 8000C0M5 apparatus (package I), lenses and preparations for ophthalmic treatments (package II) and other consumables for eye microsurgery (package III). A detailed description of the subject of the order specifies Annex No. 7 to the SWZ.
2. Delivery of the subject of the order on the ophthalmic operational block (I and III package) / to the pharmacy (Package II) of the Municipal Medical Center for them. dr. Karol Jonscher in Łódź (ul. Millionowa 14, 93-113 Łódź) will be implemented at the expense and risk of the contractor.
