Request for Quotation- LOAD

That the BAC of Hiyang-Hiyang Elementary School hereby resolve to post in Philgeps for the Bidding of our Communication Load Allowance for both Internet and Cellular Load that is charged to MOOE of the school per month.

That the BAC has agreed to qualify the bidders according to the following conditions:

1. Load Supplier must be Philgeps Registered

2. That this will be posted in a Public bidding in Accordance with the GOP.

3. That the Communication Load (Globe) and the Internet Load will be allocated a certain budget per month stated as follows:

a. April-900

b. May-500

c. June-900

d. July-300

e. September-580

f. October-780

g. December-610

4. That the total allocated budget for Internet and communication load (Globe) will be Php 4, 570.00

5. That the budget will be released every month.

6. That therefore, the Payment Terms for this transaction will be good as (COD)-Cash on Delivery.

7. That the winner of this bidding will be the sole contractor of Hiyang-Hiyang Elementary School for Communication and Internet Load from April-December 2019.
