Application should be Made on the Prescribed Form to be Obtained from this Office on Payment in Cash of a Non-Refundable Registration Fee of Rs. 1, 000.00 Per each Item. Separate Applications should be Made for each Item. Applications Which are not on Prescribed Form will be Rejected.
