The National Meat Inspection Service (NMIS) through the Bids and Awards Committee (BAC) is inviting interested parties to submit proposal/quotation for the hereunder project:

for the Contract (ABC): PhP200, 000.00

Activities Schedule

Issuance of Eligibility Requirements and Quotation Forms September 21, 2019 - September 24, 2019

Submission of Bids/Quotations On or before September 24, 2019

Opening of Bids/Quotations Exactly 11:00AM, September 24, 2019

Quotation forms and eligibility requirements may be obtained from NMIS-BAC Secretariat, Visayas Ave., Diliman, Quezon City, Tel. Nos. 924-7980 local 227 or 228.

All proposals/quotations and eligibility requirements to be submitted must be sealed in an envelope and addressed to the BAC Chairman, DR. RONNIE ERNST A. DUQUE, National Meat Inspection Service, 2nd Floor MIDTC Building, No. 4 Visayas Avenue, Brgy. Vasra, Quezon City.

Bids received in excess of ABC shall be automatically rejected at the bid opening.

The National Meat Inspection Service reserves the right to reject any quotation/proposal, to annul the process any time prior to contract award, or not award the contract when it will not redound to the benefit of NMIS, without thereby incurring any liability to the affected bidder/s.

Terms and Conditions:

1. Quotations and other requirements shall be submitted to the Bids and Awards Committee.

2. Bidder shall submit the CERTIFIED TRUE COPIES of the following updated requirements:

a. Duly signed NMIS Quotation Form

b. PhilGEPS Certificate of Registration/Membership

c. Valid and current Mayor-s Permit/Municipal License

d. Updated Income Tax/Business Tax Return

e. Notarized with Dry Sealed Omnibus Sworn Statement

f. Landbank Account Number or other Bank Account Number

3. Quoted prices shall be inclusive of applicable taxes and any bank charges for non - Landbank Account holders and shall be firm and valid for a period of at least Sixty (60) days from the date of receipt of quotation & shall be binding upon the supplier within the period.

4. Quotations shall be compared and evaluated on the basis of the following criteria:

a. Completeness of submission

b. Compliance with instructions

c. Price

5. A Bidder who accepted a contract but failed to deliver the required goods called for in the contract or Purchase Order (P.O.) shall be ground for cancellation of the contract or P.O. issued to bidder and shall be disqualified from participating in future NMIS procurement activities. This is without prejudice to the imposition of other sanctions prescribed under RA 9184 and its IRR, against the supplier.
