REQUEST FOR QUOTATION (RFQ)

The National Housing Authority (NHA)-Region 4, through its Bids and Awards Committee 2 (BAC 2), intends to procure Training Package Provider which will be undertaken in accordance with Section 53.9 of the 2016 Revised Implementing Rules and Regulations of RA 9184.

Item No. 1 : Meals (Lunch, AM and PM Snacks)

Item No. 2 : Demo Materials

Item No. 3 : Starter Kit

Location: Tanay, Rizal

Approved Budget for the Contract (ABC) : Seventy Thousand Pesos Only (PhP 70, 000.00)

Source of Fund: GAD Fund

Contract Duration: Ten (10) days upon receipt of Purchase Order (P.O)

Interested Service Provider/s are required to submit the following documents:

a) Mayor-s/Business Permit

b) PhilGEPS Registration Number; and

c) Income Tax Return

Award of contract shall be made to the lowest quotation, which complies with the Items description. The Service Providers offer or quote above the Approved Budget of the Contract (ABC) shall be automatically disqualified.
