Purchase/Acquisition of Fuel for the Maintenance of Municipal Vehicles & Heavy Equipment

Please quote your lowest price on the item/s listed below, subject to the

General Conditions on this page, stating the shortest time of delivery and

submit your quotation duly signed by your representative not later than ten

(10) days upon receipt of this form in the return envelope attached herewith.

NOTE:

1. Delivery period within ten (10) calendar days:

2. Warranty shall be for a period of six (6) months for supplies

& materials, one (1) year for equipment, from the date of

acceptance by the procuring entity

3. Price validity shall be for a period of thirty (30) calendar days.

4. Payment for goods shall be subject to 5% withholding on EVAT and

1% on Income Tax & payment for services shall be subject to 5%

withholding on EVAT & 2% on Income Tax
