Provision of Health Maintenance Services for 320 Personnel for the for the Period April 1 to December 31, 2020 with the following specifications:



a) Must be hospital based service provider;

b) Pre-existing illness and all surgical procedures and treatment covered up to maximum limit inclusive of diagnostic examinations. Physical Therapy shall be covered up to 12 sessions per medical condition that requires rehabilitation and/or recommended by the physician.

c) Covers work and non-work related accident caused injuries;

d) Private room with phone, cable tv, refrigerator and air condition for employees.

e) No incremental charges during involuntary Room Upgrade for the first 72 hours. Member can upgrade up to suite room if no private room is available;

f) P 160, 000.00 maximum coverage per illness. Philhealth shall be applied first then the remaining amount shall be covered by the Provider up to the maximum as stated.

g) Full coverage of anti-rabies vaccine for the first ten (10) incidents;

h) With EENT and with at least two (2) accredited physicians in different field of specialization except for those medical specialization/association that do not allow their members to be accredited by any health care provider (e.g. Cardiologist, Endocrinologist);

i) In and Out-patient treatment, diagnostic examinations and professional fees with non-affiliated physicians, specialists, hospitals and laboratories are allowed subject to 100% reimbursement of the actual fee if not affiliated or only one (1) affiliated physician or facility is provided.

j) All treatment (latest modalities and traditional method) are covered up to plan limit;

k) Reimbursement of more than P2, 000.00 must be processed at a maximum of one (1) month from the date of submission of all the requirements and for P2, 000.00 and less must be processed within ten (10) working days from date of submission of all requirements;

l) Ambulance service from pick-up point (any hospital, home or scene of accident) to any hospital up to P3, 000 per conduction per member will be subjected for reimbursement;

m) District-s retained company physician must be accepted in all private local hospitals and shall be accredited as coordinator;

n) Referral slips for consultation and diagnostic request forms shall be made available at BACIWA Clinic and shall assign Coordinators in accredited local hospitals with alternates in times of their unavailability;

o) Effectivity date of new members (Principal A) should be on the date appearing on the Appointment;

p) Premium should be inclusive of tax;

q) Submit quarterly utilization not later than the end of the month following the close of a quarter.

r) Request is good for 320 employees but payment depends on the actual availment (or actual number of permanent employees);

s) Service covers complete annual check-up (On Site - BACIWA Office) of all members and shall include the following:

1) Chest X-ray

2) Complete Blood Count (CBC)

3) Urinalysis

4) Fecalysis (Stool Exam)

5) Pap Smear - Female

6) Electrocardiogram (ECG)

7) Fasting Blood Sugar (FBS)

t) Service include Accidental Death and Death (AD & D) insurance benefits for the principal amounting to P30, 000.00 for natural death and P60, 000.00 for accidental death. Payment shall be made within 1 month upon receipt of claim documents.

u) Coverage is inclusive of admission kit which includes but not limited to the following: calibrated glass, spoon and fork, tissue paper, soap and face towel, thermometer, pillow, urinal, bedpan, etc.;

v) Must include services like assistance in the wellness program, medical community services (e.g. Blood letting, medical mission, Health Fora and the likes) and free immunization (vaccine and administration) of flu vaccine for all members not later than June 30, 2020. Schedule of flu vaccine administration should be minimum of Five (5) working days in BACIWA;
