TERMS OF REFERENCE

INFLUENZA VACCINE





I. BACKGROUND



Employee Relations, which is one of the general functions of the Personnel Welfare Division, is defined as an organization`s effort to prevent and resolve problems arising from situations at work through policies and programs. Part and parcel of the Employee Relations Program (ERP) is to provide a holistic wellness program for the employees of the water district.



Part of our health and wellness program is the administration of vaccines as provided for in the MCWD Medical Plan. Flu vaccine, as one of the vaccines covered with the latter, is administered to employees whose symptoms show weak resistance and who are susceptible to chronic illness like cough, colds, fever and etc. For objective implementation, consent from the employee were solicited through the distribution of the consent forms. The result were then accounted which serves as basis for the quantity in the procurement of this vaccine.



This is another initiatives that helps to address the threat of influenza, which is a potentially serious disease that can lead to hospitalization and sometimes even death. Eventually, this health promotion program can help and alleviate health risks and productivity in the workplace.



II. SPECIFICATION:

" Quadrivalent flu vaccine 0.5 ml adult dose ( with medical services )

" Quantity : 240 pieces

" Amount : 600.00 per vaccine

" Inclusive of Tax



III. SERVICES AND REQUIREMENTS:



" Audio - Visual presentation ( flu vaccination discussion )

" Flyers or posters

" Two (2) days event ; employees are to be scheduled by MCWD medical clinic

" One (1) doctor ; one (1) nurse each day

" Vaccination materials :

- Vaccination card

- Cotton balls

- Alcohol

- Band aid

- Trash bag

" Brochure / Drug literature





IV. REPEALING CLAUSE

All existing internal practices which are inconsistent with the foregoing provisions of this Terms of Reference (TOR) are deemed repealed.
