(i) DTI/SEC/CDA Registration Certificate;

(ii) Latest Mayor's/Business Permit;

(iii) Latest Income Tax Return/Business Tax Returns; Or PhilGEPS Platinum Registration

(iv) PhilGEPS Registration Number or PhilGEPS Cerificate of Registration;

(v) Omnibus Sworn Statement (duly notarized with reference 2016 Revised IRR of RA9184) (Winning Bidder)

(vi) Professional License/CV Consulting Services (Signed every page of CV)

(vii) Terms of Reference (Signed every page)

(viii) Statement of the prospective bidder of all its on going government and private contracts, including contracts awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract to be bid.

1. Prices are inclusive of taxes, delivery and other incidental costs.

2. Delivery schedule is December 05, 2018.

3. The deadline for submission of quotation shall be on Nov. 19, 2018, 11:00AM. The bidders shall send their duly accomplished form together with the eligibility.
