2. Latest Mayor Permit issued by the city or municipality where the principal place of business of prospective bidder is located

3. TAX CLEARANCE FOR BIDDING as per E.O. 398, s. 2005 reviewed and approved by B.I.R and attached C.O.R

4. PLATINUM PHILGEPS CERTIFICATE (PLATINUM MEMBERSHIP)

5. Statement of all on-going contracts attached with Notice to proceed and P.O for On-Going Contract including contracts awarded but not yet started, if any, whether similar or not in nature and complexity to the contracts to be bid

6. Statement of all Completed Government and Private Contract

Statement of Single Largest Completed Project attached Award Notice, in which is 50% to the TOTAL CONTRACT TO BE BID, Certificate of Completion, P.O, Charge Invoice and O.R similar to the contract to be bid.Except under conditions provided for in Section 23.4.13(The prospective bidder must have completed within the period specified in the Invitation to Bid and those value adjusted to current prices using the Philippine Statistic Authority consumer price indices, must be at least 50% of the ABC. Use the appropriate form as specified in BIDDING DOCUMENTS
