5. Place the RFQ in a sealed envelope and type the following details on the face of the envelope:

6. Deadline of submission of RFQ: 5 days upon receive of RFQ;

7. Information relating to the examination, evaluation & comparison of price quotation shall kept confidential & shall not disclose to any other party except to those officially concerned until award of contract.

8. Item/s delivered must have warrantied for unit replacements, parts, labor or other services.

9. Price validity shall be for a period of three (3) months

10. Quoted prices must be inclusive of taxes and other charges or fees and shall not exceed the (ABC).

11. Transactions w/ DOH-TRC shall mean compliance by the winning bidder with the bid and delivery requirements before the issuance of check payment
