REQUEST FOR QUOTATION

Please quote your lowest price (price must be inclusive of tax) on the item/s listed below, subject to the General Conditions, stating the shortest time of delivery and submit your quotation duly signed by your representative:

Note: 1. Bidder shall DOWNLOAD and QUOTE on the attached Request for Quotation (RFQ) form.

2. All entries must be typewritten or written legibly.

3. Warranty shall be for a period of six (6) months for supplies & materials, one (1) year for equipment from date of acceptance.

4. Price validity shall be for a period of thirty (30) calendar days.

5. Bidder shall attach original brochures (or Certified photocopy) showing certifications of the product being offered.

6. BIDDER SHALL SUBMIT PHOTOCOPY OF MAYOR-S/BUSINESS PERMIT, PHILGEPS REGISTRATION NUMBER, INCOME/BUSINESS TAX RETURN AND OMNIBUS SWORN STATEMENT AFTER DETERMINATION OF THE LOWEST QUOTATION AND BEFORE APPROVAL OF THE AWARD BY THE MEDICAL CENTER CHIEF (MCC)/HEAD OF THE PROCURING ENTITY (HOPE).
