REQUEST FOR QUOTATION

The Local Government of Badoc, through its Bids and Awards Committee (BAC), will undertake a Small Value Procurement for the -œProcurement of Gasoline, Diesel and Oil- with Print accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184.

Name of the Project : Procurement of Gasoline, Diesel and Oil to be used in the different Offices of the Municipality of Badoc, Ilocos Norte.

Approved Budget for

the Contract : One Hundred Ninety Nine Thousand Pesos and 00/100 (199, 000.00)

Specifications: : See attached Annex -œA-

Delivery Term : Within Three (3) days from receipt of the Contract

Interested suppliers are required to submit their valid and current Mayor-s/Business Permit, Income/Business Tax Return, Philgeps Registration Certificated and duly signed Price quotation form (Annex -œA-).

Submission of quotation and eligibility of documents at BAC Office, Municipal Hall of Badoc, Ilocos Norte, Quotation and eligibility documents must be submitted in 2 copies each in a sealed envelope.

Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above the other terms and condition stated in the price quotation form.

Any interlineations, erasures or overwriting shall be valid only if they are signed or initiated by the bidder.

The BAC reserves the right to accept or reject any or all quotations and to impose additional terms and conditions as it may deem proper.
