2.0. Pre-Qualification Requirements
Interested Contractors are Invited to Submit the following Compulsory and Mandatory Documents
I. Evidence of Company Registration with Corporate Affairs Commission (Cac), and of Memorandum of Understanding (Mou) in Case of a Joint Venture, including Cac Registration Forms co7
Ii. Certified True Copy of Memorandum and Article of Association of the Company
Iii. Corporate Profile to Include List of past Projects, Cvs and Copies of Credentials of Company and Technical Staff Indicating Qualification, Year(S) of Cognate Experience and Certificate of Registration with Relevant Professional Bodies
Iv. Evidence of Company Current Tax Clearance Certificate for the Last Three (3) Years (2016-2018), Duly Stamped:
V. Photocopy of Vat Registration Certification
Vi. Current Company’S Audited Account for the Last Three Years (2016-2018) Duly Stamped by a Professional Accounting Firm/Audit Sowing Annual Turnover;
Vii. Evidence of Compliance with Industrial Training Fund (Itf) Amendment Act 2011 by Inclusion of Compliance Certificates from Itf; Valid till December 31st, 2019
Viii. Evidence of Tax Clearance of One (1) or Two (2) Directors of the Company
Ix. Evidence of Compliance with Pension Reform Act, 2004 by Inclusion of Current Clearance Certificate from National Pension Commission (Pencom); Valid till December 31st, 2019
X. Evidence of Compliance on the Database of Contractors, Consultant’S N Service Providers by Inclusion of the Registration Report (Irr) Issued by Bureau of Public Procurement (Bpp) in Line with Provision of Section of the Public Procurement Act App, 2007.
Xi. Evidence of Nigeria Social Insurance Trust Fund (Nsitf) Compliance Certificate Valid till 31ts December, 2019
Xii. Original Bank Statement for the past Twelve (12) Months
Xiii. Satisfactory Bank Reference Letter and Document Demonstrating Availability of Funds/Liquid Assets and/or Credit Facilities from the Banker(S) to Successfully Complete the Contract
Xiv. Verifiable Evidence of Similar Jobs Successfully Executed or being Executed within the Last Five (5) Years, Indicating Types, Cost, Location and Evidence of Award an Completion;
Xv. List of Verifiable Essential Equipment in Full Working Condition to be Used for the Project, List as Owned, Hired or Leased;
Xvi. Company’S Registered Address, Functional Contact Address, Gsm Phone Numbers, and E-Mail Addresses,
Xvii. Reference Letter from Three (3) Previous Client Similar Projects, Satisfactorily Completed to be Included in the Pre-Qualification
Xviii. Original Copies for Document would be Requested to be Made Available for Sighting when Required
Xix. A Sworn Affidavit, Certifying that:
A. The Document Submitted for the Pre-Qualification Exercise are not only Genuine, but Correct
B. The Director(S) Seeking for Pre-Qualification had Never Convicted by Any Court of Law
C. The Company is not Bankrupt,
D. None of the Officers of Uniosun is a Former or Present Director of the Company
Xx. Evidence of Payment of Non-Refundable Tender Fee as Indicated below
Xxi. Insurance – All Risk Insurance Cover
Xxii. Contractor must be Able to Finance the Project up to 50% Complete before Any Payment could be Made by the University
Xxiii. Provision of Project Performance Bond (Ppb)
All the Requirements are to be Arranged in the Order above:

3.0. Verification Claims
Interested Contractor’S must Note that Uniosun Reserves the Right to Verify Claims Made in the Pre-Qualification Document Submitted by Them.

The Fee should be Paid in Bank Draft and Addressed to Osun State University, Osogbo

5.0. Submission of Documents
A. The Two Pre-Qualifications with All the Required Supporting Documentation should be Neatly Packaged, Bound and Forwarded in a Sealed Envelope and Marked Pre-Qualification and Tender Lot at the Left Corner
For Example: Pre-Qualification and Tender for the Procurement and Installation of Laboratory Equipments (Lot 10)
B. The Two Copies of Tender’S Documents Collected from Registrar’S Office, Osun State University Osogbo must be Sealed in an Envelope Marked at the Left Corner.

The Company Submitting the Pre-Qualification and Tender Documents should Write the Name Mobile Phone Number O Contact Person and the Name of the Firm at the Bottom Left Corner of the Four Envelopes.

The Pre-Qualification and Tender Documents must Reach the University not Later than 4:00 p.m. On Friday 4th October, 2019

Please Note that:
I. The Pre-Qualification and Tender Documents will be Opened at a Date to be Announced Later to All Contractors Who Submitted Bid Documents
Ii. All Those Who would have Submitted Their Documents or Their Representative by the Deadline will be Invited to the Meeting for Opening of the Pre-Qualification and Tender Documents
Iii. Only the Tender Documents of Pre-Qualified Contractors will be Opened
Iv. Contractors are Expected to Adhere Strictly to All the Instructions as Non-Compliance may Constitute a Ground for Disqualification
V. Notwithstanding the Submission Made, Osun State University, Osogbo is Neither Committed nor under Any Obligation to Include Any Contractor on the Final List of Pre-Qualification Contractors or Any Form of Contract to Any Contractors
