1. Request for Quotation
2. Latest Mayor's/Business Permit
3. PhilGEPS Registration
4. Latest ITR and BIR Certificate
RFQs must be duly received by the BAC Secretariat through manual or online submission on or before November 29, 2021 (Monday) at 1:00 PM at the Office of the City Planning and Development Coordinator, 2nd Floor, West Wing, City Hall Building, City of San Fernando, La Union. Late RFQ shall not be accepted. Opening of RFQs shall be done after submission. RFQs will be opened in the presence of the BAC Member, BAC Secretariat, and End-user.
Prospective Suppliers may obtain further information from the BAC Secretariat of the City Government of San Fernando, La Union at Office of the City Planning and Development Coordinator, 2nd Floor, West Wing, City Hall Building, City of San Fernando, La Union during 8:00 am to 5:00 pm on Mondays to Fridays except for Holidays.
