2. Interested suppliers are required to submit their valid and current Mayor-s Permit and PhilGEPS Registration Certificate, Tax Clearance, Omnibus Sworn Statement and Price Quotation (Annex -œA-). State the unit price of each item and the total bid price and also state the shortest time of delivery and submit your quotation duly signed by your representative in a sealed envelope.

3. Bids received in excess of the ABC shall be automatically rejected at Bid Opening.

4. Bidders must have Development Bank of the Philippines (DBP) account.
