The Philippine Information Agency - Region 10 would like you to provide the following:

200 pcs. Polo Shirts

Cloth: Lacoste, color Red

With DOE logo (1.5" x 1.5") on left breast and E-Power Mo Logo on right sleeve

As such, the provider or supplier of known qualifications is hereby invited to submit their quotation/price proposals not later than 05 November 2018.

The service provider must submit the following requirements:

1. Mayor's permit issued by the city or municipality where the principal place of business of the prospective bidder is located.

2. BIR Registration Certificate;

3. PhilGEPS Registration Certificate
