Description : Petroleum, Oil and Lubricants

for 1 year (January 1, 2022 to December 31, 2022)

Scope of Service

1. The service provider shall provide the Petroleum, Oil and Lubricants during regular days including Saturday, Sunday and holiday.

2. The service provider shall allow issuance of petroleum, oil and lubricants and other services to vehicles that are enrolled/listed in the DILG IV-A-s official vehicle list, in all of its branches.

3. A transaction slip/receipt/invoice shall be issued every time fuel is withdrawn or other services are given.

4. The Statement of Account should be accurate with the receipt/invoice issued by the service station on monthly basis as support for monthly payments.

5. DILG IV A shall be allowed to make purchases from the service provider on credit subject to the presentation of approved Gas Slip duly signed by the authorized signatories whose names appear on the credit application attached hereto and forming an integral part hereof.

6. The service provider shall honor only purchases covered by duly accomplished Gas Slip;...
